چکیده

Additional references are provided by the authors to support their claims. However, the Hashemi and Mehravaran study reported results from only 23 subjects, which is likely underpowered to detect small changes in the posterior surface. Ciolini and Belin investigated a larger dataset but reported amean difference between preoperative andpostoperative changes in the posterior surface without a statistical analysis, so the differences in the posterior surface that weremeasured are difficult to interpret. Both references reported a statistically significant difference between the 2 devices used to make themeasurements. The authors also suggest that the statistically significant posterior surface steepening reported by Smadja et al., which occurred in proportion to the amount of anterior tissue disruption during myopic LASIK and regressed during the 1 to 3 month postoperative interval, is “minute and clinically irrelevant.”Wedisagreewith this characterization of the conclusions since the study demonstrates with yet another imaging technology the tendency of photoablative severing of stromal lamellae to produce peripheral corneal thickening and central posterior surface steepening in the early postoperative period. In conclusion, consistent changes to the posterior surface after refractive surgery have been reported with multiple tomographic devices, although the magnitude of the changes differs by device. These changes decrease over time and are associated with a normal postoperative outcome of a safe procedure. They are also consistent with our proposed biomechanical response to refractive surgery.dCynthia J. Roberts, PhD, William J. Dupps Jr, MD, PhD

Brillouin microscopy of collagen crosslinking: noncontact depth-dependent analysis of corneal elastic modulus.

PURPOSE Corneal collagen crosslinking (CXL) is designed to halt the progression of keratoconus and corneal ectasia by inducing corneal stiffening. However, it currently is difficult to monitor and evaluate CXL outcome objectively due to the lack of suitable methods to characterize corneal mechanical properties. We validated noncontact Brillouin microscopy to quantify corneal mechanical properti...

Keratoconus.

Not too long ago, the options for treatment of keratoconus were limited to contact lens wear or penetrating keratoplasty. Times have changed, which is reflected in 4 articles in this issue that have some bearing on current investigation and treatment options. One hundred fifty years ago, Dr. John Nottingham provided the first detailed description of keratoconus, with further observations by his...

Riboflavin/ultraviolet-a-induced collagen crosslinking for the treatment of keratoconus.

PURPOSE In animal eyes, a significant increase in corneal biomechanical stiffness has been found after collagen crosslinking by combined riboflavin/ultraviolet-A (UVA) treatment. The aim of the present study was to evaluate the clinical usefulness of riboflavin/UVA-induced collagen crosslinking for bringing the progression of keratoconus to a halt. DESIGN Prospective, nonrandomized clinical p...

Collagen Crosslinking for Keratoconus

In this issue of JOVR, Derakhshan and co-authors 1 describe the results of crosslinking for patients with early keratoconus. Collagen crosslinking is a recently described technique of corneal tissue strengthening using riboflavin as a photosensitizer and ultraviolet A (UVA) radiation to promote the formation of intra-and interfibrillar covalent bonds by photosensitized oxidation. 2 To date, it ...

Protective Effects of Soluble Collagen during Ultraviolet-A Crosslinking on Enzyme-Mediated Corneal Ectatic Models

Collagen crosslinking is a relatively new treatment for structural disorders of corneal ectasia, such as keratoconus. However, there is a lack of animal models of keratoconus, which has been an obstacle for carefully analyzing the mechanisms of crosslinking and evaluating new therapies. In this study, we treated rabbit eyes with collagenase and chondroitinase enzymes to generate ex vivo corneal...
